Requesting Sacramental Records
If you need a copy of a Baptism, First Eucharist, Confirmation, or Marriage certificate from St. Mary’s Basilica, please follow the appropriate instructions:
Sending a Certificate to Another Parish?
If you are requesting that your sacramental certificate be sent directly to another parish:
- Download the Sacramental Records Request Form
- Send completed form to Gina Fernandez
- There is no charge for this service.
Requesting a Certificate for Personal Records?
If you are requesting a certificate for your own records:
- Download the Sacramental Records Request Form
- Return completed form in-person to Gina Fernandez
- A government-issued photo ID is required to verify your identity.
- There is a $25.00 service fee for personal record requests.
